Rate and Payment

RATE

We accept cash, Visa, MasterCard, HSA, or FSA cards at the time of service. Our fees are based on time spent with you and the treatment performed during your appointment. The fee ranges are as follows:

$120.00 for initial evaluation + treatment (about 60 mins) 

$125.00 for 75 minute treatment

$100.00 for 60 minute treatment

$75.00 for 45 minute treatment

Cancellation Policy: Your appointment is scheduled specifically for you. You will be expected to give a 24 hours notice for all cancellations, except in the case of an emergency. There will be a charged $25 for any missed appointment with less than a 24 hours cancellation notice. (Our answering machine is always on.)

INSURANCE

OT Life, Inc. is an out-of-network provider for all insurance plans. We have elected to remove ourselves from a in-network provider/contracted status with the insurance companies.

How do I get reimburse by my insurance company?

We are considered as “out-of-network provider” or “non-participating provider”. We will provide “superbill” as your receipt at the time of service and payment. You should be able to find a claims form on our insurance company’s website, print it out and submit it along with your “superbill” that we provide for you.

The amount of reimbursement your receive will vary according to the terms of your insurance policy. Some companies may reimburse you at 80%, some at 60%, some at 40%, and some may not reimburse you at all. We cannot make guarantees or estimates regarding what reimbursement your plan allows.

In order to provide superbill, we will need a physician’s referral with a medical diagnosis and prescription that read as “Occupational Therapy to Evaluation and Treat”.  Please bring the prescription with you at your first appointment.
